United support services prOVIDES RELIABLE innovations WAIVER CARE SERVICES
NC Innovations is a Medicaid waiver program for individuals with intellectual disabilities and other developmental disabilities. It helps individuals to live a more independent life in their own home or the home of a Direct Suppart Staff. The NC Innovations waiver offers an array of services and support options based on the Individual Support Plan (ISP)
These services are designed to assist individuals of all ages to remain in their community and to live as independently as possible. Individuals who may not currently qualify for Medicaid may qualify under this waiver. The NC Innovations Waiver enables individuals to play an essential role in deciding how to plan, obtain, and sustain commumity-based services for themselves. This helps them to live fuller, more independent lives.
United Support Services provides the following services under the NC Innovations Wavier:
Community Living and Support
Become more independent
Leam how to connect with others in your community
Leam skills such as shopping and banking : Extra support or healin and satety reasons.
Manage eating, bathing, dressing, personal care, hygiene and other daily activities
Community Networking
Find volunteer work
Take integrated classes
Explore hobbies and interests
Leam public transportationPay for membership to these activities
Respite
• Provides family caregivers relief from caregiver responsibilities
Residential Supports - Alternative Family Living (AFL)
Support in daily lite activitics in an out-of-home placement
Leam how to stay healthy and safe in the community Leam new skills and do more things for yourself
Go to community activities
Supported Living
Support in living in a home that you own or rent
Learn new skills and do more things for yourself
Manage eating, bathing, dressing, personal care, hygiene and other daily activities
Supported Employment
Find a paying job in an integrated setting
Leam new job skills
Start your own businessSupported employment can be provided long-ten for ongoing job support.
